Designing a questionnaire
Think carefully about what you want to find out from the survey and design your questionnaire accordingly. Moreover, use a mixture of closed- ended questions and open- ended questions. Last but not least, keep your questionnaire short and easy to understand.

Planning—Conducting a survey
1. Come to class with prepared question lists on exercise habits.
2. Discuss the questions within groups of four to revise the questions and decide what to be included in the questionnaire.
3. Conduct the survey and come to class with collected data.
Preparing—Collecting and analyzing the data
1. Collect the data from the survey by using the following as a sample. (Done before class)
2. Discuss in groups. Analyze the statistics, figure out the possible causes of the situation, draw conclusions and offer suggestions or possible solutions.
Producing—Writing a report
1. Each student takes the responsibility of writing about one or two questions on the questionnaire and puts down what have been discussed beforehand.
2. Exchange the drafts within group members and do some proof-reading. Ensure the logic between the sentences is correct and revise language mistakes if necessary.
Presenting—A presentation on exercise habits
Each group come to class with PPT and deliver a presentation on exercise habits, focusing on the following aspects.
