In the past year, Jeanne Raitt has had many roles while working with medical students at the KU School of Medicine-Wichita. She’s been the mom of a 5-year-old son with a serious illness, a patient relying on pain medication, and the daughter of a woman diagnosed with Alzheimer’s disease(阿尔兹海默症).
Raitt is among 85 Wichitans who participate in the school’s standardized patient program. She and others in the program have been trained by staff at the Wichita campus’ standardized patient center to role-play patients or family members. The sessions enable students to practice their skills in taking medical histories, communication, interpersonal relationships, and their ability to handle sensitive issues and embarrassing situations.
“We provide a safe environment for students to practice their clinical skills before they see actual patients,” explains Jennifer Brantley, assistant director of the standardized patient center.
Take the case of 55-year-old Ellen Humphries, a college professor who is being seen for abdominal (腹部的)pain. After an exam, students and professors discuss how to treat someone like Ellen, who is educated, well-known in the community, and who isn’t willing to stick to medical recommendations.
“This is a great way to give medical students hands-on learning and training,” says Raitt. “I love watching how students learn about the variety of the cases and seeing how willing they are to accept feedback from us. Plus, it’s a bonus(额外的好处) to get a little checkup now and then.”
For Natashia Gafford, being a standardized patient has provided her with reassurance that future doctors will be able to handle different situations. Gafford, who is the mother of six boys, particularly likes it when her role-playing corresponds with real-life issues she can relate to, such as advocating for her children’s health. “It puts my mind at ease as a parent to know that doctors will learn how to take parents into consideration,” she says.

Amazon sells 1,161 kinds of toilet brushes. I know this because I recently spent an evening trying to choose one of them for the bathroom in my new apartment. Nearly an hour later, after having read countless contradictory (矛盾的) reviews and considering far too many choices, I felt tired and simply gave up. The next day, I happily bought the only toilet brush the local dollar store offered.
Too many choices exhaust us, make us unhappy and lead us to sometimes flee from making a decision altogether. Researcher Barry Schwartz calls this “choice overload”. And it’s not just insignificant details like which brush to wipe the inside of the toilet with — having too many choices in our creative and professional lives can lead us to avoid making important decisions.
Understanding how and why we make decisions can perhaps help us make better choices down the line. We make poorer decisions when we are tired. It’s caused by decision fatigue (疲劳). The mind can only sort through so many choices and make so many choices before it starts to run out of steam. That’s why impulse buys (冲动型购物) like candy bars and magazines at the checkout aisle in the grocery store can be hard to resist. We’ve exhausted all our good decision-making skills.
The same goes for our workday. Making lots of decisions not only exhausts us; it can put us in a bad mood. That’s why it’s necessary to make your most important decisions in the morning rather than at the end of an exhausting day when your energy has been used up. The idiom “sleep on it” is indeed effective when it comes to making big decisions.
When we’re tired, we tend to save our energy by making choices based on a single factor like price, rather than considering all the other factors that go into making the best decision. When you’re doing this, you are acting as what researchers call a cognitive miser (小气鬼).
To conclude, letting yourself have fewer choices to choose from can help you arrive at a more creative answer.

The Neurosurgical Simulation (神经外科模拟) and Artificial Intelligence Learning Centre hired 70 medical students to perform virtual brain tumor(肿瘤) removals.
Researchers randomly assigned students to receive instruction and feedback by either an AI tutor or a remote expert instructor, with a third control group receiving no instruction. An AI-powered tutor called the Virtual Operative Assistant (VOA) used a machine learning algorithm(算法) to teach safe and efficient surgical technique and provided personalized feedback, while a deep learning system and a team of experts assessed student performance. In the other group, remote instructors watched the surgical simulations in real time and provided feedback based on the student’s performance.
The researchers found that students who received VOA instruction and feedback learned surgical skills 2.6 times faster and achieved 36% better performance compared to those who received instruction and feedback from remote instructors. Surgical skill plays an important role in patient outcomes both during and after brain surgery. VOA may be an effective way to increase neurosurgeon(神经外科医生) performance, improving patient safety while reducing the burden on human instructors. “Artificially intelligent tutors like the VOA may become a valuable tool in the training of the next generation of neurosurgeons,” says Rolando Del Maestro, senior author of the study in JAMA Network Open. “The VOA significantly improved professional skills while providing an excellent learning environment. Ongoing studies are assessing how in-person instructors and AI-powered intelligent tutors can most effectively be used together to improve the mastery of neurosurgical skills.”
“Intelligent tutoring systems can use a variety of simulation platforms to provide almost unlimited chances for repetitive practice without the limits imposed by the availability of supervision,” says Ali Fazlollahi, the study’s first author. “With continued research, increased development, and promotion of intelligent tutoring systems, we can be better prepared for ever-changing future challenges.”
