a long time ago意思为“很久以前”,作时间状语,句子用一般过去时态。
e.g. A long time ago, there lived a young girl.
【注意】其同义短语是once upon a time/long long ago。
e.g. Once upon a time, there lived an old man who had two beautiful daughters.

这是一个一般疑问句。could 是情态动词,没有人称和数的变化,后接动词原形,用来询问是否可做某事,意思为“能,可以”。

【拓展】 could也为can的过去式,用于一般过去时,后接动词原形。
e.g. Could you make a model house when you were a child
