It was late, about 10:15 p.m. Janice Esposito got off the train at Bellport, New York, went to her car and started driving home. She was so familiar with the route that she almost drove automatically: Turned left to the Station Road, then another left onto Montauk Highway, and then—bam! When Esposito’s car had just crossed the railroad tracks, it hit another vehicle and was pushed back onto the tracks. Injured but mostly shocked by the crash and by the airbags that popped up, she was stuck in the vehicle.
As it happened, Pete DiPinto was just about to go to sleep when he heard a sharp noise and saw the accident not far outside his bedroom window. As a volunteer firefighter and retired teacher. DiPinto, who was 65, fetched a flashlight and rushed out without hesitation. “Any firefighter would have done what I did.” he said. “We’re always on duty.”
At first, he spotted the other car in the accident. After making sure that the driver was all right, DiPinto looked around and discovered Esposito’s car straddling the railway tracks. And then he heard the bell ring, which signaled a train’s arrival.
DiPinto rushed to Esposito’s car and broke the window on the driver’s side. Esposito looked up at him, with her eyes glazing over. “I don’t know where I am.” she said.
“You’re on the railroad tracks,” DiPinto yelled. “I have to get you off right now!” The train was running toward them at a speed of some 105 kilometers per hour. The driver’s door cannot be opened due to the collision (碰撞), so DiPinto quickly ran to the other side and managed to open the door. He put the airbags aside, seized her arms, pulled her toward him across the passenger seat until finally got her out and walked her to safety as swiftly as possible. Several seconds later, the train crashed into the vehicle. “It was like a Hollywood movie,” DiPinto told reporters the next day.

The earliest bread may have been made from cattails (香蒲) and ferns (蕨类), which date back to nearly 30,000 years ago. This suggests that even before the rise of agriculture, humans had begun to form a type of flour that they would then use to create bread. The earliest wheat and barley-based bread developed from the Middle East by around 12,000 years ago.
One problem with the production of early bread was preservation, as bread would often go bad quickly. The solution was to change unused bread into beer, which proved to be far more suitable for storage. Beer likely made the production of bread less wasteful, as extra bread not eaten could then be made into beer.
With the development of the earliest bread, new technologies appeared to help with the baking process. This included enclosed ovens and open ovens that used mud or brick to make a hot surface on which flatbread could be prepared from a dough mix. Bread and earlier agricultural foods affected the development of many food preparation technologies, including pestles, querns, and mills (磨面机).
The production of bread led to many major changes in society, where the production and processing of wheat and barley for bread and other foods changed economies and social structures. Much labor was employed for the production of food with the rise of cities, where an increased population required bread to be made at more industrial scales in large ovens and prepared by many people. At first, people made flour by hand, often leading to coarse grains. Later, mills and large flat stones were used to make more high-quality flour.
Millet was another grain used to make bread, particularly in India and China. A form of flatbread made of millet is still the main food type in India. In China, sorghum and rice were also used to make bread, which made the consistency (粘稠度) and quality very different from wheat-and barley-based bread.

Wearable trackers and monitors (such as smartwatches) are increasingly popular.
Wearables are playing an increasing role in managing and discovering conditions like atrial fibrillation (心房颤动), the most common heart rhythm problem, For many people, the sense that they are receiving reliable, objective and personalized health data can encourage feelings of confidence, safety and assurance, especially when combined with symptom trackers or patient diaries. This may allow patients to self-manage their condition at home with their families, rather than spending time in hospital—reducing anxiety and stress. In a clinical setting, data may also encourage patients to take part in shared decision-making. Interpreting health data together with doctors or other health-care professionals, they can develop goals and action plans, including when to seek help, and when to go to hospital. Patients who understand their condition tend to report fewer atrial fibrillation symptoms.
However, a study examined the behavior and well-being of 172 people with atrial fibrillation over a nine-month period. It found the 83 people who used wearables to monitor their condition were more worried about their symptoms and treatment, with one in five experiencing “intense anxiety”. Chronic anxiety can contribute to stress. burnout and poor physical health, which in turn can exacerbate (使加剧) heart conditions.
Some people also describe being a “prisoner of the numbers”. They feel they “need to keep checking the device to know how they were doing, leading to the device dominating their lives”. The volume and frequency of notifications (通知), alarms and vibrations (震动) from wearable devices can be overwhelming and make people worry about their health. Information overload can also discourage self-management, with notifications instead prompting people to seek health advice more often than they otherwise would.
It’s a catch-22 situation: The wearable device may help you better manage your chronic heart condition, but wearing it could make you anxious—which is bad for those conditions.
If you already use a wearable device for health monitoring, it can be helpful to regularly review the data and notification settings. You may wish to discuss how you are using your device to help you self-manage your condition with your doctor or nurse. With any chronic disease, having a management action plan is important.
